Local dental offices: Please print a copy of our RX form and call 651-348-7848 to schedule a pickup.

2. Mail in

If not in the Twin Cities metro: Please print our RX and Shipping label and send postage free via USPS.

3. Submit Electronically

Intra oral scanners: You can send digital cases through your scanner. Simply search Woodbury Dental Lab on your scanner and connect to our lab. Please call us at 651-348-7848 if you have any issues locating us through your scanner.
